I have NO idea when this was written, but based on the handwriting, I'd have to say 8th or 9th grade:
General Appearance: Neat, masculine, tall, muscular
Character traits: good sense of humor, kind, smart, considerate, well mannered
Emotional Maturity: Doesn't act like a child, yet still acts young
Professional: I'd like to marry a doctor

Looks like I eventually got what I wanted. Well, as soon as the bearded eye-roller gets his medical degree. har!
